Endotracheal Tube

An endotracheal (ET) tube is a medical device used for intubation of the trachea and can be inserted either through the mouth (orotracheal) or through the nose (nasotracheal). An ET tube works as a pathway for gases (carbon dioxide and oxygen) to be exchanged between the lungs and an anaesthesia breathing system or blood and nasopharyngeal mater. ET tubes are most commonly made from polyvinyl chloride (PVC) - silicone, latex and stainless steel are less frequently used to make specialty variants. ET tubes are of 2 basic types - with cuff and without cuff. A cuff is an inflatable portion at one end of the ET tube and upon inflation this forms a barrier against the tracheal wall to prevent leakage of air and the sucking in (aspiration) of gastric content into the trachea. Plain ET tubes have limited use for mainly paediatric patients. ET tubes are also available in reinforced variants which make them kink resistant. The reinforcement is achieved by having a wire spiral lined on the wall of the ET tube. Reinforced tubes are mainly meant for surgery in the head and neck of the body where they can be easily bent to provide greater surgical access. The size of an ET tube is generally expressed in millimetres (mm) and refers to the internal diameter of a tube. Endotracheal intubation is a procedure in which a tube is placed through the nose/ mouth into the trachea (wind pipe). The mouth is used for the insertion of the tube in emergencies. This is done mainly under sedation or when the patient is unconscious. The purpose of endotracheal intubation is to ventilate the lungs and to ensure that air passes freely. There are many situations when this could be required including when the patient is afflicted with ailments such as pneumonia & cardiac arrest or when the patient is critical and perform normal respiratory functions. A mechanical ventilator can be used along with an endotracheal tube.
